Physicians Referral
We make it easy for physicians in Chino Hills, Chino, and Pomona to refer a patient for evaluation and treatment. Here's how the process works, and what we need from you to get your patient in and moving again.
Whether your patient needs an evaluation and treatment, a consultation, or any of the services we offer, we work to make the referral process stress free for your office and straightforward for your patient. We value our relationships with our referring physicians, and we treat every referral as a direct reflection of your trust and reputation.
How Our Referral Process Works
Our focus is getting your patients back to doing the activities they love, pain free. Once we receive a referral, our team reaches out directly to the patient to schedule their initial evaluation, and we keep you updated on their progress throughout their course of care. We know your staff's time is valuable, so we try to make referring a patient as simple as possible: send over the prescription or referral along with the patient's information, and we'll take care of the rest, including verifying benefits and confirming the appointment.
What to Include in a Referral
To get a patient scheduled quickly, please include the patient's name and contact information, the referring diagnosis, and any relevant history such as recent imaging, post-operative notes, or specific precautions we should know about before the first visit. A signed prescription or referral form covers what's needed on our end. California allows patients to access physical therapy directly without a physician referral, so a formal referral is not required for a patient to be seen, but a written referral helps us align our plan of care with your recommendations from the start.
Why Physicians Refer to Empower
Our therapists work one-on-one with patients from their first visit to their last, so the person who evaluates your patient is the same person treating them at every subsequent visit. We keep you in the loop on how your patient is progressing and reach out with questions if anything comes up that you should know about. If you have questions about a referral or want to talk through a specific patient's case, please call any of our three offices and our team will connect you with a therapist.
Fill it out, fax it, or hand it to your patient.
