
The Achilles tendon, at the back of the heel, is the thickest tendon in the body — and it's often the source when patients come to us with ankle pain. One of the most common questions we hear from new patients is why that pain keeps returning even after treatment.
A common pattern we see
Many patients have already tried physical therapy, sometimes just a session or two, and felt relief for a while — only for the pain to return and leave them feeling back at square one. When that happens repeatedly, it's usually a sign that the root cause was never identified in the first place.
The real culprit is often foot position
Your feet are the foundation your whole body works from. Just like a house with a weak foundation will develop cracks, feet that aren't positioned correctly put extra stress through certain areas of the body — often the Achilles tendon — leading to ongoing pain.
A common finding on assessment is that the arch of the foot is dropping too much, rolling the foot inward more than it should. Over time, that added stress makes the Achilles tendon tighter and causes more friction and irritation behind the heel, which is exactly the kind of pain that keeps resurfacing without lasting relief.
Addressing the root cause
Once the underlying foot mechanics are identified, treatment can be built around correcting them — for example, with a custom orthotic to support the arch — alongside a plan to reduce pain, and build strength and flexibility so you can return to your normal activities without the pain creeping back.
